We would like to know how the Scottish Ambulance Service took you to hospital, with or without your mobility aid, including wheelchairs and walking frames.

The information you provide will remain anonymous, and will be used to inform publications and policy work undertaken by the Scottish Disability Equality Forum.

This survey is in three parts.

In the first part, you will be asked about your mobility aid, including wheelchairs and walking frames.

In the second part, you will be asked about your experience of the Scottish Ambulance Service.

In the third part, you will be asked general demographic questions
